Application Requirements for the
12th International Week for Children with Leukemia
25 – 31 May, 2013

1. We invite from each country a child with leukemia and his/her mother to participate at IWCL. Unfortunately, we will not be able to accept applications of past participants.

In very exceptional cases, instead of the mother, the fathers’ accompaniment may be evaluated by the IWCL Committee; therefore please include a detailed explanation of why the mother may not accompany the child.

2. Participating children are expected to be 6 to 14 years old; diagnosed with childhood leukemia and in the remission treatment or treatment over-control period. Unfortunately, applications of children with Aplastic Anemia, Hodgkin’s and Non-Hodgkin’s Lymphoma Fanconi Anemia and Down syndrome will not be accepted.

3. Documents requested from the child applicant are:

3.1. Application Form 3.2. A newly dated epicrisis obtained from the hospital in English (Should be written on a letterhead; should include full contact details including e-mail address of hospital and doctor and must be signed by the child’s hematologist/oncologist) 3.3. Report obtained from the hospital and/or hematologist/oncologist who has been treating the child, stating that the child can make a long distance travel without any risk in English, (Should be written on a letterhead; should include full contact details including e-mail address of hospital and doctor and must be signed by the child’s hematologist / oncologist) 3.4. Copy of the treatment protocol, 3.5. Photo of the child at the time of treatment, 3.6. Copy of the ID card or preferably passport with photo.

4. Documents requested from the child’s accompanying mother are:

4.1. Copy of ID card or preferably passport with photo, 4.2. Document verifying that the companion is the participating child’s mother.

5. Accommodation and meals throughout the Week as well as domestic traveling expenses (such as flight from Istanbul to Antalya) will be covered by LOSEV. Participants are expected to cover their round trip flight expenses and personal expenses. It is strictly expected for participants to arrive at Istanbul Ataturk Airport on May 25th 2013 before 13:00 and depart from city of Antalya on June 1st 2013.

Applicants are expected to await LOSEV’s confirmation of participation and should inform LOSEV about flight reservations before purchasing the ticket.

6. It is the responsibility of the participants to apply for visa (if applicable). Invitation letters will be provided upon request.

7. Participants are kindly requested to bring publicity materials such as newsletters, brochures, booklets, magazines, badges etc., of their countries and their affiliated organizations (medical center where treatment is received, supporting group/association/civil society etc.) as well as symbolic souvenirs to give to other participants.
