1- Applicants should be 6-14 years old, diagnosed with childhood leukemia and should be in the remission treatment or treatment over-control period. The application of children with Aplastic Anemia, Fanconi Anemia and Down syndrome will not be accepted.
2- The participants should confirm and submit the application form to LOSEV and Turkish Embassy/Consulate by the end of April 1, 2009.
3- The participant children should submit the fallowing documents to LOSEV by the end of April 20, 2009
a) a “hospital committee report” that confirm child’s health condition (or a statement by the hematologist/oncologist with his/her full contact details including e-mail address)
b) a copy of the ID card or preferably passport (with a photo)
c) a copy of the treatment protocol
d) a health report obtained from the child’s hospital and/or doctor, stating that the leukemic child may make a long distance travel
4- The companion should be his/her mother. Companions are required to submit following documents to LOSEV and Turkish Embassy / Consulate located in their Country by the end of April 20, 2009:
a) a copy of ID card or preferably passport (with a photo)
b) a document verifying that the companion is the participating child’s mother.
5- Each country will be represented by a maximum of 2 people: 1 leukemic child and 1 accompanying mother.
6- Previous year’s participants are requested not to apply for a second time.
7- Participants are kindly requested to bring publicity materials of their countries and their affiliated institutions (Hospital, Health Center, NGO, Charity Foundation etc.) such as newsletters, brochures, booklets, magazines, badges etc., symbolic souvenirs which will be given to other participants and our Foundation as a memory of the organization.
8- Participants are kindly requested to bring traditional costumes special to their cultures to wear at the Gala Night and official ceremonies.
9- Participants are kindly requested to prepare a short speech or a presentation that they would perform at the Opening or at School Festival about the objectives and the meaning of the International Week for Children with Leukemia (optional).
10- All participants should have filled out the application form completely and returned it to the Turkish Embassy/Consulate located in their country AND should fax to LOSEV (+90 312 4450532) or send through e-mail (international@losev.org.tr) by the end of April 1, 2009 and other necessary documents April 20, 2009.
